**Fixed term contract / secondment opportunity for 2 years.** 

A new and exciting opportunity has arisen within the Nephrology Department at the Norfolk & Norwich University Hospital (NNUH). 

We are looking for a self-motivated enthusiastic young adult worker who is passionate about helping young adults.  The successful applicant will work alongside the Nephrology Multidisciplinary team to help support the emotional, psychological and social wellbeing of young adults with chronic kidney disease.  Specialist knowledge of Kidney disease is not necessary but candidates should be able to display knowledge of the challenges facing young adults with a long term medical condition.

The successful candidate would need to work flexibly to meet patient needs, some of this time would be spent in a dialysis unit working alongside patient and multidisciplinary team.

You will assist in the development and delivery of a comprehensive support service for young people and adult services in NNUH, in the care of Nephrology, a significant part of this role will be supporting patients with their dialysis.

Part time: 18 hours per week. The successful candidate would be required to work days, evenings and the occasional weekends.
