* Fixed Term / Secondment opportunity - 6 months *

This is an interesting opportunity to develop skills and experience in a post with an important central role in the delivery of patient flow and a high quality patient experience.

As post holder you will work closely with clinical and operational staff from across the Trust, and the wider health system, to deliver, promote and improve effective discharge pathways for all patients. Your role will be focused on preventing delays to discharge and ensuring patient safety and the best possible patient experience at all times.

We are looking for a highly motivated and experienced clinician, with a good grasp of discharge planning, and a keen interest in providing the best possible care to patients, who can work collaboratively and effectively to drive improvement in all parts of our Discharge service. The day to day management of the Discharge team will be the major component of this 6 month role; but there will also be some opportunity for the post holder to support discharge related project work across the local  health system

Hours of work : Full time - 37.5  hours per week
