We are looking to recruit a committed and dynamic nurse to join the Integrated Discharge Team, to support the Discharge to Assess Pathway 3 in the Continuing Healthcare Team.

This is a very exciting and unique role working within a specialist team, in both the acute and the community environment, visiting patients in a care home or their own home.

In this role, you will gain an understanding of the continuing healthcare framework and assessment process, giving the candidate further career opportunities.

With guidance from the Operational Lead and D2A pathway manager, as post holder you will :

- promote and implement the NHS Continuing Health Care Process and develop the discharge to assess pathway

-identify assess and Co-ordinate services to support the safe discharge and transfer of patients with complex needs.

- case manage and act as key worker for complex patients’ prior to discharge. Actively promote the implementation of Single Assessment Process, National Service Framework for Older People and Vulnerable Adults Process.

Hours of work: part time 30 hours per week
