An exciting new role has arisen within the integrated discharge team to support with the data collection, basic analysis and understanding of the effectiveness of changes within the discharge services. The role will be to work collaboratively with the integrated discharge team in supporting improvements in service and provision provided by the team. This will be key in supporting the smooth running and effectiveness of our service and ensure that all relevant information is correctly inputted to support future objectives of the team.
The successful candidate will also play a vital role with our national reporting of Delayed Transfer’s of Care, alongside internal reporting around performance and metric achievements.
As post holder you will be required to use their experience of data extraction, report creation and their analytical skills to support the matron and senior nurse and integrated discharge team to provide an accurate, efficient and pro-active service that meets the information needs of the Trust and the wider health economy. This will involve working with minimal supervision and the ability to meet tight deadlines.

Hours of work : Full time - 37.5 hours per week, to include occasional weekend and bank holiday working
