This role is incorporated into the integrated discharge team working collaboratively throughout the Trust with patients of all ages with complex discharge requirements. Supporting the Trust in facilitation of flow and working as part of a ward team in supporting all discharges and discharge planning.

This is a developmental opportunity starting as a band 3 progressing to a Band 4. 

To independently and actively monitor the multidisciplinary management and discharge plan for all patients in designated wards and take actions to expedite the processes, avoid delays and improve patient experience. Early escalation to Senior Matron and complex discharge team. This is a critical success factor in the role of reducing length of stay for all patients.

Hours of Work: Full Time 37.5 hours per week 

Will include weekend and bank holiday working, 7 day working.
